Can someone guide me on how to make a mantle like this photo?  Thank you!

The walls with stone are straight forward so I assume you can model those.

• Place a doorway for the fireplace opening and size it accordingly (you can use the doorway casing for a material to fill any space between the fireplace and the surround).
• place a fireplace, adjust size and position
• Place a Fireplace Surround from the Eldorado Stone Catalog (The Giada)

While the Giada is close, if you want to match that exactly, a molding polyline would do the trick.
